No 4spds in A-vans from the factory. There was a light duty manual (A-9??) and a heavy duty manual (A-745) available. You could go to an automatic.
I know a guy up in Alaska that had an A-108 with a 440/4spd that he installed himself. He had the shifter mounted on the dogbox and made up cables to shift the trans.
