Flat glass. Laminated. Most glass shops should be able to duplicate the pattern. Looks like you have a nice ride, so you may consider the blue band on the top.
FYI - Walker eliminated the gasket (back beaded with typical urethane instead) and butt glazed his with some clear silicone at the seam. Looks real good + you don't have to buy the gasket if it's dry and cracking up.

I had mine made using 20 percent tinted i'm real happy with that..
